Enclosed please find a Sponsorship Form for Particle Accelerator V, our fifth annual suicide awareness and prevention benefit concert.  Last year, many generous local businesses reached out and sponsored Particle Accelerator IV.  With their help the event raised over 5000 dollars, an all-time high for Particle Accelerator.  We hope that your support will help us make our fifth concert the most successful year yet.

Particle Accelerator is an annual benefit concert with two main goals:

1. Raise money in order to support United Services, Inc. of Dayville, CT in their mission of providing “an effective response to the youth, family and adult social and behavioral health needs in its community.” Some programs we have supported are the 24 Hour Emergency Hotline and the Mental Health First Aid Training course.

2. Provide information and support for depression and suicide prevention, while promoting music and civic engagement as a healthy alternative to drug and alcohol abuse among young adults.

Particle Accelerator V will be held at Rotary Park in Putnam, CT on June 11, 2011.  The Pledge Form may also be completed online at our website, www.particleaccelerator.org. Aside from monetary donations, raffle donations are welcomed and greatly appreciated.
